When I started thinking way too much about zombies: Umm...after I watched Dawn of the Dead when I was seven...
What started my zombie fixation: see above...
Favorite zombie book: Plague Zone by David Wellington
Favorite zombie movies: 28 Days later, Dawn of the Dead(the remake), and Quarantine
Favorite quote in any zombie movie: Jim: No, no. No, see, this is a really **** idea. You know why? Because it's really obviously a **** idea. -28 days later. (if only they didn't waste half of the movie with him saying "hello?" it would've been so much better.)

My Survival Plan:

My location: Altus, Oklahoma (or Olustee, Oklahoma, depending)
Is my plan dependent on zombies in your home town, or anywhere? not necessarily. It would work for any major evacuation circumstance.
Will I be staying in my home or headed somewhere else? Somewhere else.
If I'm heading out, how do I plan to get there? well, i have a huge van in my front yard...It would take much more than anything remotely humanoid to get into that monster.
Got anyone I want to take with me? My family, and the people I'm living with. Also my friend John, because that guy is insanely intelligent, and as paranoid as I am. A few others too, though only if i have the time and resources.
How will I communicate with the outside world, or do I plan to go the hermit route? If I have all the people mentioned above + any uninfected survivors along the way, them hermit it is.
How am I planning to get food and water? Using the supplies on hand, I would also raid the smaller stores to avoid getting caught in a crowd, and live off the land.
What’s my weapon of choice? Swords. Do you need to reload that thing? Swords never need to be reloaded...that's why their the weapon of choice. I'll have secondary weapons for ranged attacks, but nothing beats something that won't run out of ammo. Gotta plan for that? For the secondary weapons, yeah. Besides the ammo I'd already have, there's a small shop that sells ammo. Not too many people even know about it, so it'd be pretty easy to swipe some. (or buy some beforehand...)
Got a survival philosophy? yeah. Don't die... (Pull together, work as a pack, and everyone will live longer?) but mainly don't die...
Am I willing to hook up with other survivors? It depends on the situation, but if i find others who need help, I'd probably let them go with us.
Is my plan based on a book/movie/blog/scientist other survivors may have heard of? Umm..Not sure. I'm still planning a lot of it, so i have no idea. The one I'm developing one based on some from the book mentioned above, and a few other random sources.

Upon viewing the basic map layout, I have a few ideas for planning purposes.
First, The road network appears limited which is a good thing for maintaining either quarantine or limiting avenues of attack. The primary feature that I find of significant interest is the rail line between the two town.
Is this an active line? If so, that would be most beneficial if one of those mile long trains bearing all possible goods just happened to be in the vicinity when SHTF. Otherwise, it can be a very secure interior line of operations.

Natural features (such as the small rivers) also aid in the defense.
Major population centers (Amarillo, Wichita Falls, Oklahoma City) appear far enough away to build up defensive lines before mass exodus into your area.

The main difficulty I see for you is getting enough people to back up such a plan of action prior to being overrun with refugees and then zeds.

Altus AFB is primarily a training base with some airlift capability (they teach airlift and air refueling) that will not likely be called away. This will be a strategic resource for the post-zed world.
